Credit Card Authorization Form

All customers are now required to keep their credit card on file with Y.E.S. Upon completion of services rendered an invoice will be emailed to you, which is due on receipt. At the time of payment, you may request the balance be paid with your card on file. If you would like to use an alternative form of payment, we also accept cash, checks or wire transfers (we do not accept American Express). No charges will be billed to your card unless an open balance is not paid within 30 days of invoicing.
